A year at maintenance

At a steady maintenance dose, a year of Zepbound at $449 costs $5,388 if every refill is on time, and up to $8,388 at 10–15 mg if refills are late. A year of the Wegovy pen at $349 costs $4,188 ($3,888 in a new patient’s first year with the two $199 fills); Wegovy HD is $4,788. For pills, the top doses cost $299 a month on both channels, $3,588 a year. Our arithmetic; prices rise with dose on both.

Both channels sell the FDA-approved product at the manufacturer’s price with no membership. Telehealth services that resell the same drugs, such as Ro and Hers, add a membership on top. Compounded alternatives are not the same products; see the pricing database.

LillyDirect: who it fits

LillyDirect fits you if your prescriber recommends tirzepatide or Foundayo and you can refill inside the 45-day window. The 2.5 and 5.5 mg Foundayo reductions end December 31, 2026. Full LillyDirect review · Zepbound cost without insurance.

NovoCare Pharmacy: who it fits

NovoCare fits you if your prescriber recommends semaglutide: the Wegovy pen price does not rise across the standard doses, and there is no refill-timing penalty on its offer page. Full NovoCare review · Wegovy cost without insurance.

Same need, different moleculesZepbound (tirzepatide) and Wegovy (semaglutide) are different drugs. In SURMOUNT-5, tirzepatide produced 20.2% average weight loss against 13.7% for semaglutide. Results vary, and no medicine guarantees weight loss. Both manufacturers require a valid prescription, and eligible Medicare Part D enrollees can pay a $50 copay under the GLP-1 Bridge from July 1, 2026 to December 31, 2027.

Questions to ask both, in writing

  1. Which molecule and dose does my prescriber recommend, and what will the maintenance dose cost?
  2. Does my insurance cover either drug, and does a savings card apply?
  3. Am I eligible for the Medicare GLP-1 Bridge?
  4. Who will see me for follow-up, since neither pharmacy provides clinical care?

Frequently asked questions

Is LillyDirect or NovoCare cheaper?

At the starting dose they tie: Foundayo 0.8 mg on LillyDirect and the Wegovy pill 1.5 mg on NovoCare are both $149 a month (October 15, 2026). For injections, the Wegovy pen is $349 at standard doses while Zepbound is $449 at 7.5 to 15 mg with on-time refills, so Wegovy costs less at maintenance.

What is the cheapest FDA-approved GLP-1 without insurance?

As of October 15, 2026, the lowest self-pay price for an FDA-approved GLP-1 weight-loss medicine is $149 a month: the Wegovy pill at 1.5 mg through NovoCare, and Foundayo at 0.8 mg through LillyDirect, where 2.5 mg is also $149 under a reduction that runs through December 31, 2026. Prices rise with dose.

Do LillyDirect and NovoCare include a doctor visit?

No. Both are pharmacy channels that fill a valid prescription. You need a prescriber, such as your own clinician or a telehealth service, which may charge separately. Neither price includes a clinical visit, so add your prescriber’s fee when comparing with telehealth programs.
