NexLife tirzepatide injection: prices, plans and evidence status

Direct answer · October 15, 2026

As of October 15, 2026, NexLife's compounded tirzepatide injection costs $169 a month month to month, or $159, $149 and $139 a month on 3-, 6- and 12-month plans, with the same price at every dose and no membership fee, according to its plan page on nexlife.us.

Compounded tirzepatide is not FDA-approved and is not Mounjaro or Zepbound; a clinician decides whether you qualify and at what dose.

Prices by plan length

These are the four plan tiles on NexLife's tirzepatide page. The savings column is our arithmetic and matches the “Save” badges NexLife prints.

Because NexLife prices every dose the same, the table is the whole price list. You do not pay more when your prescriber moves you from a starting dose to a maintenance dose.

What 12 months costs

Paying month to month for a full year costs 12 × $169 = $2,028. The 12-month plan costs 12 × $139 = $1,668, a difference of $360. The 6-month plan works out to $894 for six months, and the 3-month plan to $477 for three. Those totals hold at any dose, since NexLife does not charge by milligram.

What is included

NexLife lists these inclusions in the monthly price:

Lab work is not stated as included on any NexLife page we read. There is no membership fee and no separate visit fee.

Billing, cancellation and refunds

How you are billed. The plan pages say “Plans continue monthly unless canceled” and that savings are “applied at checkout on eligible multi-month plans.” They do not say in one sentence whether a multi-month plan is charged each month or collected up front, though they do say unshipped months are refunded if treatment stops. Confirm the charge at checkout.

Cancelling. Plans renew until you cancel. Cancellations “take effect after 30 days, so you may receive and be charged for one additional shipment,” unless NexLife Medical Group approves stopping sooner for medical reasons. NexLife's FAQ calls plans cancellable “at any time”; plan around the 30 days the plan page states.

Refunds. If you are not prescribed, the authorization charge is refunded. Once medication ships, the order is final. On 3-, 6- and 12-month plans, unshipped months are refunded if treatment stops.

Guarantee. NexLife's written policy refunds program and membership fees, not medication, only if you lose less than 5% of your starting weight after 180 days and meet its conditions. The banner on the plan page (“lose up to 10% in 6 months or get your money back”) does not match that policy.

Evidence status

The efficacy data for tirzepatide come from the FDA-approved products. In SURMOUNT-1, tirzepatide 15 mg reduced body weight by 20.9% on average at 72 weeks (treatment-regimen estimand). In SURMOUNT-5, tirzepatide produced 20.2% weight reduction versus 13.7% for semaglutide. Those trials used Lilly's approved drug, not a compounded preparation, and no trial has tested NexLife's product. Results vary and no program can promise weight loss. Read our SURMOUNT-1 summary for what the trial did and did not show.

How it compares

On October 15, 2026, the 28 compounded tirzepatide programs in our index published lowest all-in prices from $86 to $280 a month, with a median of $144. NexLife's $139 12-month rate is just under that median. It is not the floor: Mondae ($126) and Eden Health ($129) charge less month to month with no membership, and twelve programs publish a lower figure in all. Brand Zepbound through LillyDirect starts at $299 a month for 2.5 mg. See the pricing database for every row, or the broader NexLife tirzepatide pricing page for microdose and ODT.

Frequently asked questions

How much is NexLife tirzepatide per month?

As of October 15, 2026, NexLife's compounded tirzepatide injection is $169 a month month to month, $159 on a 3-month plan, $149 on a 6-month plan and $139 on a 12-month plan, according to its plan page. There is no membership fee.

Does NexLife tirzepatide cost more at higher doses?

No. NexLife's plan page says “Same price at every dose,” so the plan price does not rise when your prescriber increases the dose. A 12-month plan totals $1,668 at any dose.

Is NexLife tirzepatide the same as Zepbound or Mounjaro?

No. NexLife's tirzepatide is compounded by a pharmacy for an individual patient. It is not FDA-approved and has not been reviewed by FDA for safety, effectiveness or quality. Zepbound and Mounjaro are Lilly's approved products.

Can I cancel a NexLife tirzepatide plan?

Yes, but the plan page says cancellations take effect after 30 days, so one more shipment may be charged. Once medication ships the order is final, and unshipped months on 3-, 6- and 12-month plans are refunded if treatment stops.

Does NexLife include lab work with tirzepatide?

No NexLife page we read on October 15, 2026 states that lab work is included. The plan includes provider review, a nutrition plan, a coaching call, 24/7 support and free expedited shipping.
