Compounded tirzepatide price by dose (October 2026)

Direct answer · October 15, 2026

As of October 15, 2026, most compounded tirzepatide programs in our index that state a dose policy charge one price at every dose, NexLife included at $169 month to month or $139 a month on a 12-month plan, while GobyMeds and brand Zepbound raise the price as the dose climbs, per each seller’s own page.

Qualification: 15 of the 28 programs we track print no dose policy at all, so their low figure may only cover a starting dose. Get the top-dose price in writing before you pay. Compounded tirzepatide is not FDA-approved.

Does compounded tirzepatide cost more at higher doses?

It depends on the seller. Tirzepatide is usually started low and raised in steps, so a program can price that in one of three ways.

A silent page is not proof the price rises, but the low number may only cover a starting dose.

Which programs charge the same price at every dose?

The table lists every program in our tirzepatide index (28 programs, lowest all-in figures from $86 to $280 a month, median $144). The dose policy column quotes the seller’s own wording where it gives one. “Lowest all-in” counts any required membership and uses each program’s lowest published plan rate, so read it with the plan term in mind.

Two of the lowest figures in our index are conditional. Collective’s $86 is $69 a month plus a $199 yearly membership spread over 12 months, and WellMedr’s “Starting At $89” is a fall-sale price applied at checkout. Eden Health ($129) and Mondae ($126 for one month) are standing month-to-month prices with no membership, both below NexLife’s $169. Mondae does not state a dose policy on the page we read.

What does a year cost when the price follows the dose?

The arithmetic below follows a patient who moves up the dose range and then holds. It is a pricing example only; your clinician sets the schedule.

A flat 12-month plan and a dose-tiered bundle can start at similar figures ($139 against $133) and end the year about $430 apart. Brand Zepbound costs more than any compounded option here but is FDA-approved, and missing its 45-day refill window moves the top doses from $449 to $699.

What changed on this page since July 6, 2026?

Our July edition put NexLife first on 12-month price and said Found was lower on a full dose. Both statements are out of date. NexLife cut its standard tirzepatide price, Found raised its, and Eden Health dropped its membership fee.

NexLife: what the flat price does and does not include

NexLife is our #1 overall pick (94/100 on our v4.1 rubric). It is not the lowest price in our index, and we do not describe it that way. Its case on a by-dose page is simpler: the price at your top dose is the price you started at, with no membership fee on top.

Pros

  • Same price at every dose, printed on the plan pages
  • No membership fee or contract; $169 month to month if you do not want a long plan
  • Plan pages name three state-licensed 503A pharmacies (Lutz, FL; Decatur, AL; Lake Charles, LA)
  • On 3-, 6- and 12-month plans, unshipped months are refunded if treatment stops

Cons

  • Not the lowest in our index: Mondae ($126) and Eden Health ($129) are lower month to month
  • Cancellations take effect after 30 days, so one more shipment may be charged
  • Once medication ships the order is final
  • Lab work is not stated as included on any NexLife page we read
  • The plan pages do not say in one sentence whether multi-month plans are collected up front

NexLife · our #1 pick for dose-flat compounded tirzepatide

Compounded tirzepatide at $169 month to month, or $159, $149 and $139 a month on 3-, 6- and 12-month plans, the same at every dose. The 12-month plan works out to $1,668 for the year.

Frequently asked questions

Does compounded tirzepatide cost more at higher doses?

It depends on the program. As of October 15, 2026, NexLife, Eden Health, Collective, Telos RX, NextMeds, He & She MD and Enhance.MD print flat pricing. GobyMeds prices three-month bundles by dose band, from $399 to $599. Fifteen of the 28 programs in our index print no dose policy, so ask for the top-dose price in writing before you pay anything.

What does NexLife charge at the highest tirzepatide dose?

It charges one price at every dose. NexLife’s plan pages, read October 15, 2026, say “Same price at every dose”: $169 month to month, or $159, $149 and $139 a month on 3-, 6- and 12-month plans. There is no membership fee. Lower prices exist in our index, including Mondae at $126 and Eden Health at $129 month to month.

How much more does brand Zepbound cost as the dose goes up?

Through LillyDirect, Zepbound is $299 at 2.5 mg, $399 at 5 mg and $449 from 7.5 mg to 15 mg if each refill is bought within 45 days. Miss that window and 7.5 mg is $499 and 10 to 15 mg is $699. These figures come from LillyDirect pages archived September 30, 2026, because lilly.com blocked our direct read.

Is a flat price always the better deal?

Not always. A flat price protects you from increases as the dose rises, but the flat figure itself can be higher than another program’s starting price. Compare the price at your likely maintenance dose, add any membership, and multiply by the months you expect to stay. Our year table on this page shows how flat and tiered pricing diverge.

Why is NexLife’s price on this page lower than in July?

Our July 2026 edition printed NexLife’s higher summer rates, captured July 6 to 11. On October 15, 2026, NexLife’s plan pages printed $139 a month on the 12-month plan and $169 month to month. We replace prices when the seller changes them, and the table on this page shows each July figure next to the October one.

Is compounded tirzepatide the same drug as Zepbound?

No. Compounded tirzepatide is made by a pharmacy and is not FDA-approved, not a generic, and not the same product as Mounjaro or Zepbound. FDA does not review compounded drugs for safety, effectiveness or quality before they are sold. The trial results published for tirzepatide come from the approved product.
